New Patient Registration

To register, call into the surgery at any time during normal opening hours and ask at reception for a registration form.

The receptionist will need to know your permanent address in order to ascertain whether you are living in our practice area. You will be required to complete the form in full and in order to do this you will need to be able to provide the name and address of your last doctor, your place of birth as well as your date of birth, your last address and, very importantly, your NHS number (not your NI number). Without these details the registration form will not be able to be processed.

New Patient Health Checks

As part of the registration process we offer new patient health checks, which are carried out by our nurse by appointment only. We would advise all our patients that is is good medical practice to check your blood pressure at least every five years from age thirty onwards.

Pre-booked Telephone consultations are also available with a doctor or nurse at the surgery.

Simple self testing kits are available through one of our doctors or practice nurses.
