What is involved?

First of all we will ask you for suggestions about what areas of the service at valley road surgery you think we should be asking people registered at the practice about.

Then we will ask you about the survey we design before sending it out.

When we have the results of the survey we will ask for your input as we plan what changes we can make in response to the results.

We plan to do most of this by email or post.

Any comments made will be seen by some of the doctors, but without any names attached.

When you give your details on the form attached we will only use them for this purpose and they WILL NOT be passed to anyone else.

If you decide you do no longer wish to be involved please let us know at any time.
